Complex Care Services

To manage effectively the care for clients who require complex care provision:

  • Effective response to the request for a Complex package
  • Full assessment and planning for discharge or takeover of package
  • Appointed Lead nurse to every package
  • Full care plan to include MDT protocols, risk assessment and competencies
  • All staff trained to client specific competencies and signed off by full care team
  • Ongoing Assessment and review of care plan, monthly reviews
  • Responsive office team allocated consultant
  • Effective evaluation of all services
  • Weekly time schedules for all clients
  • Rapid response to changes in care
  • Work with respite service to ensure safe effective care
  • Service will liaise with MDT as required

 

CHC Provision

This service is designed provide ongoing CHC packages of care CHC packages the aims are to ensure:

  • Effective response to the request for a Continuing Healthcare Package
  • Ongoing Assessment and review of care plan and full risk assessment
  • Responsive office team allocated consultant
  • Effective evaluation of all services
  • Weekly time schedules for all clients
  • Rapid response to changes in car
  • Service will liaise with MDT as required
  • Backfill when current agencies cannot support

Personal Health Budget Services

Anyone who chooses to have a personal health budget can have support to decide how they would like to use their budget to meet their health and wellbeing needs.
We offer a range of support services for you to choose from enabling you to take as much or as little control of your care and support, as you want.
Managed Account Service - PHBS

We offer a Managed Account Service, which deals with the administration of your budget, including:

  • Take on the role of employer and pay employees wages directly into their bank account on your behalf.
  • Pay invoices that are sent to us
  • Support to recruit and employ your own Personal Assistants or Carers
  • We can supply staff if for any reason your staff cannot attend
  • Keep records of the money paid into your account and the money paid out.
  • Prepare and submit your account for audit as and when required by the council or NHS
  • Advising you of low or insufficient funds in your account
  • Manage your care training requirements for staff (at a charge)
  • Manage cover in emergencies
